KHPR Scholarship Applications

Complete the application form and use it as the cover page for your KHPR Scholarship submission packet. Submit your scholarship application packet by 5:00 p.m. CST on Friday, March 8, 2019 to:

Include all requested information for each scholarship in this packet except for the three reference forms, which must be mailed directly to the Scholarship Committee by the persons providing your references.

Applications which are incomplete or not typed will be rejected.

Application Packet

Application Form: Use the completed Application Form as the cover letter of your Application Packet.

Reference Forms: Arrange to have exactly three SCARFs (Scholarship Committee Applicant's Reference Form) completed and mailed from individuals who are familiar with one or all of the following: 1) your academic performance, 2) work or professional experiences, and 3) character.  The combination of all three SCARF references must address the three areas listed above.  SCARFsform is attached below.

SCARFs: must be current and must have been completed within six months of application.

Transcripts: Include your unofficial transcript(s) of all college course work from all universities/colleges.

Resume: Include an updated version of your resume.

Essay: Submit a typed, one-page essay describing your career goals, honors, awards, and organizations, plus your academic and/or service contributions to the field (be specific in what you did and for how long), which may include leadership, volunteer experience, and professional development opportunities you have experienced.  Check the Specific Scholarship Requirements for possible additional information to be included.

In evaluating applications, the Scholarship Committee will consider the GPA, résumé, references (SCARFs), writing ability, and the record of, and potential for, contribution to the particular scholarship's field.  Preference will be given by committee members to applicants who have made contributions firstly to their program in the Department of KHPR and secondly to the profession and KHPR in general, as demonstrated in the essay.
